That list is Amazon's, from the News_Amazon announcement on the Seller Forums. The first five are about the product itself and match Articles 9(5) to 9(7) and 16(3) of Regulation (EU) 2023/988. The last three are the listing duties in Article 19.
Only the last three are things Amazon can see. The first five still apply whether or not Amazon asks for them.
Amazon's Selling Partner API changelog of 18 November 2024 names the affected marketplaces: Spain, France, Belgium, Netherlands, Germany, Italy, Sweden and Poland. It says selling partners who sell non-food products in EU-based stores had to prove GPSR compliance by 13 December 2024.
The Ezra_Amazon announcement says that for listings in the Amazon UK store you do not need to submit any information to Amazon, but you must submit it for listings in EU-based stores.
That is a statement about Amazon's fields, not about the law. The same announcement says the regulation covers the EU including Northern Ireland, so a UK-store listing that ships to Northern Ireland is still within the regulation's reach.

Emma_Amazon's Seller Forums post sets out the steps Amazon's GPSR team gave for uploading warning and safety information or safety attestations in bulk. The short version:
Amazon says a submission may take up to 14 working days to review, that documents and images which do not meet its guidelines are rejected, and that uploading something does not guarantee it will be displayed.
Amazon uses a single flag called gpsr_safety_attestation. Its two pages describe it differently, and neither explains it further.
The API changelog describes the attribute as "Boolean. True if product is exempt from safety attestation". The bulk-upload post tells sellers who want to provide a safety attestation to enter "Yes" in the gpsr_safety_attestation column.
The regulation has no concept called a safety attestation. It is an Amazon field. If you are unsure which way the flag reads for your listing, ask Seller Support before setting it, because it is a declaration you are making about your own product.
Nothing in Regulation (EU) 2023/988 requires a certificate or an attestation for a general consumer product. That part is Amazon policy, not law.
The changelog says to use the getDefinitionsProductType operation to pull the JSON schemas carrying these attributes, and warns that full programmatic updates through legacy XML feed types will override GPSR information the seller uploaded, so only partial updates should be sent that way.
The Ezra_Amazon announcement says non-compliant listings risk removal and ineligibility for flash sales, and tells sellers who will not continue selling in EU stores to remove the affected products from inventory.
Behind that sits Article 22(11), which requires online marketplaces to suspend, for a reasonable period and after a prior warning, traders that frequently offer non-compliant products.
Amazon's announcement lists eight items: existing labelling and traceability, an EU Responsible Person, the Responsible Person's, manufacturer's and importer's details on the product, a type, batch or serial number, safety information and warnings on the product in the language of sale, and then the Responsible Person and manufacturer details, a product picture and the warnings in the online listing.
Amazon says you do not need to submit any information to Amazon for listings in the UK store, but you must for EU-based stores. That is about Amazon's fields. The regulation itself applies in the EU and Northern Ireland, so a sale into Northern Ireland is still covered.

Amazon's guidance is Catalog, then Add products via upload, then generate a spreadsheet for one product type and one store in that store's language, fill the purple Safety & compliance columns (documents, PS01 to PS06 images, the attestation), upload it and check the Spreadsheet upload status tab. Repeat per store.
